I, the undersigned, acknowledge that I understand and agree to the following:

I agree to give you a minimum of 48 hours notice for all cancellations or changes of scheduled appointments. I understand that missing an appointment without prior cancellation or with cancellation less than 48 hours, will be charged to me at the current full rate.

During our session, I will be alone and in a safe environment that is free from outside distractions (e.g. phones, pets, people, etc.) Further, I understand that I cannot record any portion of our session, unless prior arrangements have been made with you. You will, however, receive a recording of the hypnosis part of the session as appropriate.

Additional Conditions: Our sessions will be on zoom, so for an optimal experience you’ll need headphones, and a handsfree device, ideally a computer.

Fees and Payment Schedule

Individual sessions are payable to Carol Benton, C.Ht. at the end of our session by cash, check, credit card, Venmo or PayPal. Packages are in advance, unless otherwise agreed.

The first session is 120 minutes for $225 if separate from a package.

My continuing hourly rate is $150.

Introductory package rate is $450 which includes the first 2-hour session and 2 additional 1-hour sessions.
